PLEASE RETURN ALL COMPLETED FORMS BY AUGUST 21st
Print off, fill out and drop off at the church main office in a sealed envelope labeled “Tree of Life Forms.” Completed forms may also be mailed to: Tree of Life Christian Preschool Director: Forms 819 E. Silver Spring Dr. Whitefish Bay, WI 53217
There are also copies of these forms at the church office if you prefer to pick up paper copies and fill out.
