The forms listed below are for current students (2025-2026).
If you are looking to submit paperwork to register your child for the 2025-2026 school year, please follow this link for more information. We still have some open spots in the Rainbow Room.
PLEASE RETURN ALL COMPLETED FORMS BY AUGUST 22nd.
Print off, fill out and drop off at the church main office in a sealed envelope labeled “Tree of Life Forms.” Completed forms may also be mailed to:

Tree of Life is a non-profit ministry of the United Methodist Church of Whitefish Bay. We welcome families and children of all faiths as it is not a prerequisite to belong to the church to attend Tree of Life Preschool.
